Where can I get the Rebate form? Will it be included with a phone purchase from Atlas.?

Hi Gregory, there is a link to the rebate form on the Atlas website here. My guess is that if you purchased the set directly from Atlas by phone you will need print out and complete the form then send it along with a copy of your purchase invoice to Atlas. You can find the instructions on the Atlas website.
